Position: Full-Time Registered Nurse Case Manager, Level 2
Territory: Northwest Sacramento, including Natomas, Arden/Arden-Arcade, North Highlands, Del Paso Heights, and Elverta.
Role Overview:
As a Registered Nurse (RN), Level 2 Home Health Case Manager, you will leverage your exceptional clinical skills to design, implement, and manage comprehensive care plans aimed at promoting wellness and preventing hospital readmissions for patients in their homes. In this leadership capacity, you will oversee two Nurses responsible for executing your care plan. Your duties will include conducting extensive patient assessments such as Start of Care (SOC), evaluations, resumption of care, and discharge planning.
Your role will also involve guiding patients and their families/caregivers in wound care, blood draws, tube feedings, tracheostomy care, Foley catheters, and managing congestive heart failure, among other responsibilities.
