As a Social Worker within our interdisciplinary team, you will play a crucial role in delivering a variety of direct and indirect services to patients and their families. Your responsibilities will include identifying psychosocial issues, developing effective treatment plans, and ensuring a smooth discharge process.
In this role, you will collaborate closely with patients, families, and health care teams to address and resolve psychosocial concerns, all while adhering to Montefiore's mission and the ethical standards of the National Association of Social Workers.
Key responsibilities include:
- Conducting psychosocial assessments and screenings for patients requiring social service intervention.
- Creating, implementing, and validating discharge and treatment plans in collaboration with patients and families.
- Maintaining professionalism and confidentiality in accordance with hospital policies.
- Participating in departmental programs focused on quality improvement and continuing education.
- Demonstrating leadership through psychosocial education and conflict resolution among interdisciplinary staff.
- Staying informed on legal and ethical issues related to end-of-life care and family violence intervention.
