We are currently seeking a passionate and proactive Nurse Case Manager to become a pivotal member of our forward-thinking virtual health team. In this role, you will be instrumental in coordinating patient care, addressing complex health challenges, and facilitating seamless communication among healthcare professionals, patients, and their families. By collaborating with a multidisciplinary team, you will develop tailored care plans that encompass both conventional and holistic methodologies, ultimately improving patient health outcomes.
This position requires availability for the following shift: Monday to Friday, from 8:00 AM to 5:00 PM.
For candidates in Denver - please be prepared to commute to our Denver office located at I25 and Belleview.
For candidates in Scottsdale - you will be required to be onsite at our client's office near Frank Lloyd Wright Blvd and the 101.
Key Responsibilities
- Conduct comprehensive assessments of patients to create, implement, and manage personalized care plans.
- Communicate effectively with patients, healthcare providers, and families about health management strategies and available resources.
- Provide education and support, empowering patients to manage their health and understand their medical conditions.
- Collaborate with interdisciplinary team members to assess progress and modify care plans as needed.
- Accurately document all interactions and care plans within our electronic medical record system.
- Advocate for patients to guarantee they receive necessary services and resources.
- Maintain a compassionate and patient-centered approach in all interactions.
