As a key member of our healthcare team, the Nurse Manager for Whole Person Integrated Care (WPIC) will oversee the comprehensive delivery and evaluation of nursing services for medically and psycho-socially vulnerable clients, particularly those experiencing homelessness or residing in shelters and supportive housing. This role involves leading a dedicated team focused on assessing client needs, developing tailored care plans, and coordinating services to stabilize individuals in their current living situations while preventing unnecessary hospital admissions.
The Nurse Manager will collaborate closely with fellow WPIC nurse managers and engage in vital clinical case conferences to ensure high-quality care for our city's most complex populations. Your leadership will drive innovative interventions aimed at addressing common health issues within homeless communities, ensuring that vulnerable clients receive the appropriate level of care.
Key responsibilities include:
- Conducting thorough assessments, planning, implementing, and evaluating nursing care while utilizing patient outcomes data.
- Collaborating with multidisciplinary teams to identify health care challenges faced by homeless populations, developing innovative care strategies.
- Leading WPIC clinical and administrative meetings to discuss case management and quality improvement initiatives.
- Participating in quality assurance programs and managing quality control activities to enhance patient safety and care standards.
- Overseeing the hiring process, providing supervision, and conducting performance evaluations for a diverse team of healthcare professionals.
- Maintaining compliance with nursing policies and safety standards, including infection control and hazardous waste management.
- Coordinating the emergency response plan and ensuring all staff are informed and prepared.
- Managing occupational health programs and ensuring compliance with departmental protocols.
