YOUR ROLE
As an Oncology Nurse Navigator at Thyme Care Inc., you will serve as a crucial clinical resource for our members and their care networks. Your responsibilities will encompass triage, support, and education throughout their cancer journey via phone and email.
Under the supervision of our Nurse Team Lead, you will perform comprehensive clinical assessments, manage member health, and facilitate discussions regarding end-of-life care. Your primary focus will be on engaging with members, addressing clinical concerns, and effectively managing escalations. Unlike roles focused on transitions of care or complex oncology, this position emphasizes symptom management and care coordination, rather than managing admissions and discharges.
Shift Details: 11:30 AM - 8:00 PM EST.
In your initial three months, you will:
- Acquaint yourself with Thyme Care’s systems and tools, making at least 20 member calls per day.
- Work closely with Nurse leaders and Medical Directors to ensure compliance with clinical protocols and best practices.
