Compensation: $21.75 per hour
Position Overview
This role entails providing personalized support to HOPE clients by identifying and addressing barriers that hinder their successful integration into the community. The position may include travel and in-home training and intervention for clients placed in community employment settings. You will collaborate with clients and their families to set training goals and track their progress.
Key Responsibilities
The following duties outline the essential responsibilities of this position:
1. Conduct individualized assessments, training, and follow-ups in clients’ homes and communities according to their Individual Service Plan (ISP).
2. Provide travel training to clients to enhance their mobility and independence.
3. Ensure adherence to safety protocols and confidentiality standards.
4. Serve as a program monitor for assigned clients, maintaining accurate documentation including incident reports, attendance, billing, and case recordings.
5. Perform task analysis and time studies for wage evaluations as required by the Department of Labor.
6. Document client progress and conduct quality control evaluations.
7. Maintain comprehensive case records for all assigned clients, participate in case reviews, and assist in goal planning.
8. Foster professional relationships with clients, families, and stakeholders to support client success.
