As a Registered Nurse, the Patient Care Coordinator not only delivers direct patient care but is also responsible for the assessment, planning, coordination, implementation, and evaluation of individualized care plans for assigned patients. This role is pivotal in ensuring quality outcomes through exceptional clinical practices, with responsibilities tailored to specific settings or assignments.
Key Responsibilities:
- Plan, coordinate, direct, and deliver appropriate clinical care to patients in designated care environments.
- Collaborate with an interdisciplinary team to coordinate ongoing care, focusing on cost-effective strategies to enhance wellness and improve patient lives.
- Develop and provide tailored education for patients, families, caregivers, and staff, serving as a resource for continuous learning.
- Act as a communication bridge among patients, families, caregivers, and the healthcare team.
- Facilitate comprehensive care coordination for patients, including follow-up visits, referrals, testing, treatment, and long-term care.
- Engage in departmental, hospital, or professional committees and quality improvement initiatives.
- Ensure that clinical practices and patient care align with organizational policies and procedures.
- Perform additional duties as assigned.
