The Personal Care Assistant plays a crucial role in our team, offering essential support to our participants while also engaging in the care planning process as an integral member of our interdisciplinary team (IDT). Under the supervision of the Day Center Manager, the Personal Care Assistant is responsible for delivering safe and effective personal care services to participants at our WelbeHealth day center.

Your daily responsibilities will include:
- Providing hands-on assistance with activities of daily living (ADLs) to WelbeHealth participants, including escorting, toileting, bathing, dressing, grooming, laundry, and assisting with physical transfers.
- Measuring and documenting participants’ vital signs as instructed.
- Acting as a medical escort when necessary, aiding participants in boarding and exiting transportation vehicles.
- Monitoring activities within the day center to ensure participant safety.
- Assisting with meal service, ensuring participants receive the correct meals and providing feeding assistance when required.
