Pine Acres Healthcare & Rehabilitation Center is a local skilled nursing facility actively seeking a dedicated Physical Therapy Aide/Assistant to join our compassionate team. We offer full-time, part-time, and PRN positions to suit your schedule.
Responsibilities:
- Collaborate effectively with the therapy teams and the Rehab Department.
- Conduct evaluations and maintain thorough documentation.
- Provide insightful recommendations that enhance communications and assessments.
- Deliver therapy in accordance with established treatment plans and prescriptions.
- Administer treatments aimed at improving patients' abilities and overcoming their challenges.
- Manage your workflow independently, ensuring timely documentation for individual and group treatments in our electronic records system.
- Familiarity with Patient-Driven Payment Model (PDPM) is preferred.
- Ensure therapy treatments align with Medicaid and Medicare billing guidelines.
- Valid New Jersey License.
- Experience in long-term care, particularly in a Skilled Nursing Facility.
- Strong organizational, interpersonal, and communication skills.
- Ability to effectively manage a diverse patient population with varying personalities and abilities.
- Creative problem-solving skills.
