About the job

At WelbeHealth, we are committed to providing exceptional patient-centered care through our interdisciplinary team (IDT). Our team comprises both clinical and non-clinical members who work collaboratively to enhance the well-being of our participants.
The Physical Therapist plays a vital role within the IDT, concentrating on the development, coordination, and delivery of rehabilitative and restorative physical therapy services. You will be instrumental in helping participants achieve maximum independence and safety in their daily lives.
What Makes This Role Unique at WelbeHealth:
- Manage lower patient volumes, allowing for more dedicated care – say goodbye to rushed appointments.
- Build meaningful, long-term relationships with participants rather than providing brief rehabilitation.
- Engage in collaborative efforts with other therapists and healthcare providers.
- Enjoy a balanced work/life environment – no excessive documentation outside of work hours.
We Value Our Team Members:
- Comprehensive medical insurance (Medical, Dental, Vision).
- Work/life balance with 17 days of personal time off (PTO), 12 observed holidays, and 6 sick days.
- 401K savings plan with matching contributions.
- A competitive compensation package that includes both base salary and performance bonuses.
- And a range of additional benefits!
Your Daily Responsibilities Will Include:
- Conducting thorough Physical Therapy assessments at our facilities and, as needed, in participants' homes to identify their care needs, preferences, and goals.
- Actively participating in IDT meetings to develop personalized care plans for participants.
- Implementing and documenting therapy interventions as outlined in participants' care plans.
- Administering both active and passive exercise routines, muscle re-education, gait training, and functional training for daily living activities, utilizing various treatment modalities.
- Supervising Therapy Assistants, providing essential guidance and training for implementing Functional Maintenance Programs (FMP).
- Educating staff, participants, and their families on the proper use and care of therapeutic equipment.
