Job Summary
We are seeking a dedicated Primary Care Nurse Practitioner / Physician Assistant (NP/PA) to join our team and provide on-site care to residents in long-term care environments. This full-time role is perfect for a provider who values meaningful patient relationships and thrives in a collaborative care setting.
Responsibilities
- Deliver primary care services to residents in skilled nursing and long-term care facilities
- Conduct physical examinations and assess medical histories
- Diagnose and manage both chronic and acute health conditions
- Order and analyze diagnostic tests and lab results
- Create and modify individualized care plans as needed
- Prescribe medications and monitor their effectiveness
- Collaborate with facility staff and interdisciplinary teams
- Educate patients and their families regarding treatment options and care objectives
- Promote a safe, respectful, and supportive environment for residents
Requirements
- Current and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
- State-specific licensure in good standing in Pennsylvania
- At least 6 months of experience in long-term care or skilled nursing
- Excellent communication, clinical, and critical thinking abilities
- Genuine passion for elder care and patient-centered healthcare
- Must be capable of lifting a minimum of 20 pounds and standing/walking for up to 6-8 hours a day
