We are actively seeking a Medical Director and Attending Physician based in Detroit, MI, to serve our local skilled nursing facilities within a committed, mission-driven care community.

Your Responsibilities:
- Act as the attending physician for patients in each assigned skilled nursing facility (SNF).
- Work in collaboration with NPs/PAs to manage daily patient care and clinical supervision.
- Deliver high-quality, patient-centered primary care, addressing both acute and chronic conditions.
- Engage in Theoria’s Chronic Care Management and Transitional Care initiatives.
