As the Program Manager for Integrated Care, you will play a pivotal role in nurturing and overseeing high-performing teams consisting of Nurses, Personal Support Workers (PSWs), and Allied Health professionals. Your leadership will promote a collaborative, innovative, and learning-oriented environment that is in line with Spectrum Health Care’s Mission, Vision, and Values.
This position requires you to ensure smooth program operations by utilizing data-driven insights to enhance service delivery, maintain quality, and achieve significant outcomes. A vital component of your role will be to work closely with external health system stakeholders to develop and implement innovative programs that facilitate seamless care transitions across the healthcare system.
- Drive program development and design clinical workflows that support innovative care models while effectively managing relationships with key internal and external stakeholders.
- Recruit and onboard team members, ensuring they possess the requisite skills, attributes, and training; support ongoing development initiatives and the cultivation of best-practice champions.
- Guide and mentor the team to consistently uphold high standards of care quality.
- Collaborate with operational leadership to implement and sustain exemplary practices.
- Tackle quality, safety, and risk issues, developing improvement plans as necessary.
- Monitor and report on key performance indicators, initiating improvements or process changes as needed.
- Enhance client outcomes through evidence-based practices and efficient resource utilization.
- Integrate client satisfaction plans, promptly addressing any concerns.
