GENERAL JOB DESCRIPTION: Under general supervision, the 2830 Public Health Nurse performs vital professional nursing duties.
Key responsibilities include, but are not limited to:
- Providing comprehensive public health nursing services focusing on maternal, child, and adult health; conducting home visits for birthing and postpartum clients and families; interpreting treatment plans and assisting clients in understanding them; delivering health education regarding nutritional needs, hygiene, and safety measures; guiding clients post-delivery on newborn care and how to access emergency medical assistance; assessing the health, safety, and developmental needs of newborns while supporting clients in infant care.
- Delivering direct nursing services to at-risk populations, including children, adults, and older adults with disabilities, chronic illnesses, mental health issues, or substance use disorders; facilitating access to medical care and linking/referring clients to appropriate human services, community resources, and safety net programs; maintaining case supervision and accurate records of services provided.
- Administering nursing care related to infection control and non-infectious diseases; conducting interviews with cases and suspected infections at home, within the assigned district, or at the district health center; interpreting medical advice for patient care and instructing patients on self-care.
- Utilizing community organization concepts and collective impact strategies while working with communities to address their health needs; employing community engagement and quality improvement strategies to enhance health outcomes for clients and communities.
- Conducting case finding in diverse settings, maintaining nursing documentation in health records, and ensuring information is accessible to medical and administrative staff as required.
- Coordinating and facilitating community health and public health nursing services for assigned clients in line with program guidelines, ensuring follow-up assessments and service linkages.
- Performing thorough health, psychosocial, educational, and environmental assessments/screenings in clients' homes, both within and outside San Francisco City and County.
- Coordinating care for complex medical and health needs of populations, including accessing and organizing services identified in the individualized service plan.
- Preparing and maintaining individual client records; utilizing required forms for referrals, consents, and narrative and statistical records; compiling various statistical reports related to client outcomes.
- Assisting with health promotion and risk reduction interventions and providing health education to clients.
