Job Summary:
As a Registered Nurse Case Manager, your primary role will be to provide hospice care to clients of all ages within their homes, skilled nursing facilities, or assisted living facilities. You will coordinate care in collaboration with a multi-disciplinary team, patients, families, and referring agencies, taking full responsibility for the coordination of care. This position requires you to work independently without on-site supervision.
Essential Functions:
- Utilize scientific knowledge from social, biological, and physical sciences in the application of the nursing process.
- Exhibit knowledge and proficiency in performing foundational nursing skills as outlined in Agency policy and clinical standards, maintaining clinical competencies in accordance with home care standards, and adhering to state practice acts.
- Administer medications and treatments in compliance with Agency policy, accepted care standards, and home care guidelines.
- Conduct thorough assessments, create, and implement a detailed Plan of Care with the patient, following physician orders and Agency guidelines; document nursing interventions including patient education and adjust as patient needs evolve.
- Maintain accurate and timely documentation of patient status as required by Agency policy, addressing patient needs, nursing interventions, responses, safety measures, and educational efforts.
- Establish and nurture strong, positive relationships with referring physicians and patient family members involved in care planning.
- Ensure all interactions and communications with referring physicians comply with relevant healthcare laws and regulations.
- Provide comfort and support tailored to hospice patients’ needs.
- Collaborate with interdisciplinary teams to create and implement personalized care plans.
- Educate patients and families regarding healthcare plans, symptom management, and end-of-life care options.
- Make scheduled visits and participate in the on-call rotation as required.
