- The registered professional nurse assumes accountability and responsibility for the individualized, age-specific care of patients throughout their continuum of care, from admission to discharge. She/He delivers direct care to assigned patients and assists with additional patient care as necessary. Decisions and actions taken on behalf of patients and their families are made in an ethical manner. The nurse collects and analyzes assessment data to formulate nursing diagnoses, collaborates with patients/families and other healthcare team members, develops care plans to achieve realistic outcomes, provides evidence-based education, and systematically evaluates the quality and effectiveness of nursing practice.
Responsibilities:
- Assess and document patient status and identify care needs upon admission.
- Organize health data to ensure accuracy and accessibility.
- Identify and address problems requiring immediate attention.
- Validate diagnoses with patients/families and healthcare team members.
- Document diagnoses to facilitate outcome determination and care planning.
- Initiate and update the Problem List during admission, incorporating age-specific care principles into the plan.
- Ensure the individualized plan of care is discussed and agreed upon with patients/families.
- Begin discharge planning at the time of admission and consult with patients/families and team members accordingly.
- Identify patient education needs at admission and throughout the hospital stay.
