Key Responsibilities:
- Assess and document patient status, identifying care needs upon admission.
- Organize and maintain accurate health data for accessibility.
- Identify urgent health issues that require immediate attention.
- Coordinate interdisciplinary care effectively.
- Conduct joint home visits with a Charge Nurse.
- Validate diagnoses with patients, families, and healthcare team members.
- Document diagnoses to facilitate care planning and expected outcomes.
- Initiate and update the Problem List at the time of admission, incorporating age-specific care principles.
- Discuss and confirm the individualized care plan with the patient and family.
- Begin discharge planning at admission, consulting with patients, families, and healthcare teams as necessary.
- Identify and address patient education needs during the care process.
