Responsibilities of the Registered Nurse (RN):
- Implement care plans tailored to each patient's needs.
- Conduct home visits to clients in designated areas.
- Administer medications, provide wound care, and perform treatments and procedures as needed.
- Monitor and report any changes in clients' conditions to the Clinical or Client Services Manager.
- Follow through on and document physicians' orders accurately.
- Conduct client assessments as required.
- Engage in case management and coordination.
- Utilize state-of-the-art technology to document observations and care interventions.
Qualifications for the Registered Nurse (RN):
- A current and valid RN license in Pennsylvania.
- Previous experience in home health care is preferred.
- Strong communication skills and a compassionate approach to patient care.
- Ability to work independently and as part of a team.
- Flexibility to adapt to various patient needs and schedules.
