Your Responsibilities
As a Medical Assistant (Care Coordinator) within Evergreen's Chronic Care Management (CCM) program, you will facilitate proactive care coordination for patients with complex chronic conditions including chronic kidney disease (CKD), hypertension, diabetes, cardiovascular diseases, and other related comorbidities. Under the clinical supervision of Evergreen, you will conduct patient outreach, maintain and reinforce established care plans, ensure documentation compliance, and coordinate communication among patients, providers, and healthcare partners.
This role is primarily outreach-focused and operationally driven, requiring excellent organizational skills, effective patient communication, and the capability to manage a high-volume patient panel. Previous experience in nephrology or with CKD patient populations is highly desirable.
The program utilizes SMS as the primary channel for patient engagement, supplemented by phone outreach as needed.
While this position is fully remote, candidates must be available to work from 8:00 AM to 5:00 PM Pacific Time.
