Founded in 2020 and headquartered in Silicon Valley, Empo Health is an innovative startup that is seed-funded, backed by venture capital, and supported by the National Science Foundation. We are on a mission to develop groundbreaking in-home health monitoring solutions and are currently seeking experienced Remote Patient Monitoring LPNs on a contract basis to deliver clinical services to our partner healthcare system and its patients.
Our flagship product, the Empo Footprint, is a state-of-the-art in-home imaging scale designed to assist healthcare providers and patients in monitoring for early indicators of diabetic foot ulcers. Following a successful pilot study that surpassed all expectations, our team is diligently preparing for the launch of this world-class device alongside the Empo Remote Health Link service in the upcoming months. This position is hands-on, where you will monitor patient data, collaborate with supervising physicians, and coordinate with operations supervisors at Empo.

Job Responsibilities
This is a part-time contracted position with the potential to expand to near-full-time hours. LPNs will be contracted and managed by Empo, while working under the supervision of patients' physicians to deliver clinical services. Responsibilities include:
Monitoring foot images and weight data collected via the Empo Footprint and presented in the Empo Remote Health Link web portal on a daily basis. Escalating critical findings to the physicians and/or patients as necessary.
Conducting routine patient check-ins and adherence reminders, ensuring proper documentation is completed for each interaction. Check-ins will occur as frequently as once per month for an initial caseload of up to 50 patients.
Providing additional outreach to patients with inquiries, health changes, or issues with their Empo devices, ensuring timely resolution and accurate documentation of all communications.
Fielding patient questions and directing them to other members of the Empo team as needed.
Collaborating with patients' physicians.
