The Opportunity:
We are seeking California-licensed Registered Nurses with backgrounds in care management or population health to join our Senior Health Care Team. In this integral role, you will provide support to One Medical's senior patients throughout the care continuum via scheduled visits and proactive outreach. Your base of operations will be in an office dedicated to senior care, while also supporting patients across multiple locations.
Employment Type:
- Full-time, 40 hours per week, Monday through Friday, in-office
Responsibilities:
- Conduct independent nursing visits (both in-person and virtual) focusing on preventive care, medication reconciliation, geriatric assessments, care plan reinforcement, chronic disease follow-ups, and advance care planning.
- Empower patients to manage their own healthcare more effectively, navigate the healthcare system, and understand their medical conditions and medications.
- Collaborate with primary care providers and health navigators for follow-up care before and after clinician visits, ensuring high-quality, holistic care, especially for high-risk patients.
- Participate in case conferences and team huddles to enhance collaborative patient care.
- Coordinate with community resources such as home health services, specialists, and medical equipment suppliers to enhance patient support.
- Work with team members to identify and address care gaps, including essential screenings and preventive measures.
