BAYADA Home Health Care is actively looking for a dedicated and experienced Speech Language Pathologist (SLP) for a part-time or per diem opportunity to conduct outpatient home health visits. This role is vital in providing care for adult and geriatric patients in their own homes across the Salem community.
While prior experience in home care as a Speech Language Pathologist is advantageous, it is not a prerequisite.
As a home care Speech Language Pathologist, you will play a crucial role in a dynamic multidisciplinary healthcare team, delivering rehabilitative care and skilled nursing services that empower clients to receive the essential medical care they need while remaining in their homes.
- Conduct home visits to clients within designated geographic areas.
- Perform comprehensive evaluations to assess, diagnose, and provide therapeutic interventions for clients facing speech, language, communication, or swallowing challenges.
- Develop tailored treatment plans based on evaluations, physician directives, and set objectives.
- Continuously evaluate and update the speech therapy care plan in collaboration with BAYADA clinicians within the multidisciplinary care framework.
- Educate and guide patients, family members, or representatives on rehabilitative care and activities that promote the client's health, safety, and independent living.
- Accurately document observations, interventions, and evaluations related to patient care management and services rendered, utilizing advanced touchpad technology.
