Our dedicated team consists of both clinical and non-clinical members who work collaboratively to ensure participants receive quality primary care and social services, enabling them to live safely and independently within their communities.
As a Social Worker reporting to the Social Work Manager, you will manage a caseload of participants while closely coordinating with the IDT. Your role will involve standard social work services such as psychosocial assessments, care planning, counseling, and case management. Additionally, you will serve as a vital link between participants, their families, support networks, and care teams.

Daily responsibilities include:
- Conducting social work assessments to identify participants' psychosocial needs, preferences, and goals, while actively engaging in IDT meetings to formulate care plans.
- Implementing and documenting social work interventions outlined in participants’ care plans, such as arranging necessary resources, assisting with care transitions, and providing counseling.
- Collaborating with Primary Care Physicians and IDT members to facilitate seamless transitions of care.
- Coordinating and leading care conference meetings to enhance care coordination among participants, their support networks, and the care team.
- Assisting with discharge planning to ensure effective post-care support.
