We are seeking a dedicated temporary Occupational Therapist to join our wonderful home care domain, Zorg in de Wijk, starting February 2026, due to maternity leave. As an Occupational Therapist at Cordaan, you will be the specialist who assists (vulnerable) elderly clients in performing daily activities within their own homes. Collaborating with a multidisciplinary team of healthcare professionals, you will ensure that clients can live safely and independently at home while providing care that meets their individual needs. What We Offer You:
A six-month contract ranging from 24 to 36 hours per week, with the potential for a permanent position.
Salary in accordance with the CAO VVT FWG 55 (minimum €3,504 and maximum €5,029 gross per month based on a full-time employment).
A year-end bonus of 8.33% of your gross annual income and a vacation allowance of 8% per year.

Your Responsibilities:
As an Occupational Therapist, you will work independently in the community, assessing clients' daily functioning and identifying any barriers they may face. By proactively identifying and diagnosing issues, you can respond to clients' needs promptly. You play a crucial role in collaborating with the neighborhood team and other experts. You assist the neighborhood team in determining suitable care and contribute to enhancing clients' self-reliance. If necessary, you will utilize innovative tools to support your efforts.
In your designated area, you will establish a robust network with neighborhood nurses, teams, therapists, and other (social) community support staff. Together, you will strive to provide care that aligns closely with the clients' wishes and capabilities. Additionally, you will actively contribute to shaping a vision for self-reliance within neighborhood care and the Occupational Therapist's role in it. You will also collaborate on improving the working conditions for neighborhood team members during client home visits.
