Your Opportunity
As a Therapy Continuity Coordinator at Dreem Health, you will be a vital player in maintaining patient engagement throughout their therapy journey. This role is not merely about follow-ups; you will proactively support patients enrolled in our PAP treatment program, ensuring their ongoing success.
Reporting directly to the Customer Success Manager, your contributions will be pivotal in enhancing our recurring revenue model through timely patient outreach, proactive retention strategies, and empathetic communication. Your efforts will lead to reduced churn rates, consistent reorder cycles, and a high-quality, seamless resupply experience.
