Join New Day as a Transitional Supportive Housing Coordinator, where you'll play a vital role in empowering youth as they transition into independence. Your responsibilities will include:
Facilitating orientation sessions for youth referred by CYFD to our Transitional Supportive Housing program.
Assisting clients in finding safe and affordable housing that meets Housing Quality Standards (HQS), while ensuring all necessary documentation and rent calculations are accurately completed.
Preparing comprehensive reports for agency staff and CYFD regarding billing and compliance with regulations.
Coordinating rent payments with residents, addressing their inquiries related to tenancy, and following up on any outstanding rent issues.
Maintaining open lines of communication with property managers and program participants to clarify rights and responsibilities, resolve barriers, and formulate collaborative plans that enhance housing retention.
Establishing regular contact with youth, focusing on their housing stability, and sharing progress and challenges with CYFD staff.
Building trusting relationships with youth through a Trauma-Informed Care approach, fostering their personal growth as they transition to adulthood.
Connecting youth to additional resources and support systems as desired.
Visiting each youth’s apartment at least twice a month to provide support and facilitate communication.
Delivering services in a culturally sensitive and youth-friendly manner.
Maintaining detailed documentation for each youth according to grant and program requirements.
