Position Summary: We are seeking a dedicated Travel Acute Rehabilitation Registered Nurse Case Manager to serve as a critical liaison for coordinating exceptional medical care. This role is pivotal in providing support to both patients and psychiatric clinicians, ensuring high-quality customer service to both in-person and remote patients.
Key Responsibilities:
Operational Excellence:
- Create and maintain a positive work environment, championing a culture of engagement and connection among clinicians and staff.
- Deliver outstanding customer service by responding promptly to patient needs, effectively managing challenging situations.
- Facilitate clear communication between peers, clinicians, and patients to enhance patient care and clinician satisfaction.
Patient Support:
- Assist psychiatric clinicians with essential patient care tasks, including prior authorizations and lab orders.
- Enhance communication between patients, medical staff, and pharmacies.
- Manage incoming calls, addressing medication inquiries, refill requests, and appointment reschedules.
- Ensure accurate pharmacy information is recorded in patient medical records.
- Scan and integrate hard copy correspondence into electronic health records (EHR).
- Assist patients with telehealth access and troubleshoot Patient Portal issues.
- Respond immediately to urgent requests, triaging patient concerns as necessary.
- Support the coordination of external referrals for patient care.
Clinician Support:
- Provide administrative support to clinicians, including sending letters and faxes.
- Coordinate with clinicians regarding patient inquiries and concerns.
- Assist in the patient termination process as needed, facilitating external referrals and status updates.
