Job Summary
We are on the lookout for a dedicated and empathetic Neurology Nurse Practitioner or Physician Assistant to join our dynamic team. In this vital role, you will provide comprehensive care to patients suffering from neurological disorders such as epilepsy, multiple sclerosis, Parkinson's disease, and other related conditions. Key Responsibilities
- Create and execute personalized treatment plans for patients, which may include medication management and non-pharmacological interventions.
- Conduct regular follow-up visits to assess patient progress, refine treatment plans, and provide education and support.
- Work in collaboration with physicians and other healthcare professionals to coordinate care and ensure a multidisciplinary approach.
- Educate patients and their families on disease management, medications, and lifestyle adjustments.
- Maintain accurate and timely documentation of patient encounters and treatment plans.
- Engage in quality improvement initiatives aimed at enhancing patient outcomes and optimizing care delivery.
- Perform thorough assessments to accurately diagnose and treat patients with neurological conditions.
Qualifications
- At least 6 months of experience in long-term care or a skilled nursing facility
- Possession of an active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
