Position Summary

This role entails providing personalized support to clients, helping them identify obstacles, foster community engagement, and develop strategies to access activities and services tailored to their needs as outlined in their Person-Centered Plan and Individualized Personal Plan.

Key Responsibilities

The following are crucial functions of this position:

1. Deliver individualized support in home or community environments as specified in the consumer’s Individualized Personal Plan.

2. Collaborate closely with family members to assess needs and formulate actionable plans.

3. Create avenues for consumers to engage as valued and active contributors within their communities. Facilitate positive interactions in community settings.

4. Monitor and report on consumer progress and challenges to ensure effective support.
