Job Description:

As a Physical Therapist Assistant, you will provide comprehensive treatment in patients' homes, including skilled documentation via Homecare Homebase.

  • Deliver physical therapy services as prescribed in the care plan.

  • Complete timely clinical documentation and visit notes.

  • Effectively communicate with physicians, supervising therapists, and the interdisciplinary care team, as well as patients and caregivers.

  • Be comfortable working in all aspects of the home health environment.

  • Implement and uphold physical therapy protocols, tailoring them to specialty populations as appropriate.

  • Collaborate with the supervising therapist to manage patient functional outcomes through effective treatment.
