Position: Full Time Physical Therapist Case Manager
Territory: Alameda County: Livermore, Pleasanton
As a Physical Therapist Case Manager, you will work collaboratively with other clinicians to deliver skilled services that assess and address the needs of individuals in their homes, helping them live safely and productively. Your primary goals will be to restore function, minimize disability after illness or injury, and assist clients in achieving their maximum potential. You will evaluate and treat patients in accordance with medically prescribed physical therapy programs and may supervise Physical Therapy Assistants (PTAs) in compliance with agency and regulatory standards.
