Project Worker - Aftercare Support in Dublin
Location: Dublin
Contract: Full-Time, Specific Purpose
This role involves being an integral part of the Aftercare Team, working on a 7-day rota to provide essential care and support for young individuals residing in the project as they transition towards independent living.
Key Responsibilities:
Develop personalized care plans to guide and assist each resident in their journey to independent living.
Provide holistic support to young people, addressing their practical self-care needs, health, budgeting, and domestic skills.
Encourage and build the confidence and self-esteem of young individuals leaving care.
Advocate for residents regarding housing, social work, social welfare, education, training, and legal rights.
Collaborate with and make referrals to relevant external agencies.
Deliver individual support and key-working to project residents.
Facilitate group work sessions for the residents.
Work on a 7-day rotating schedule.
Assist in maintaining high hygiene standards within the project.
Identify and report any Health, Safety, and Welfare risks in the workplace to your line manager or Health & Safety Representative.
Maintain accurate case records and statistics.
Regularly communicate with the Project Leader regarding individual case issues.
Engage in regular supervision sessions with the Project Leader/Assistant Project Leader.
Participate in relevant training, development courses, and project evaluations.
Implement and adhere to existing project policies and procedures along with Focus Ireland's customer service standards.
