This role involves being an integral part of the Aftercare Team, working on a 7-day rota to provide essential care and support for young individuals residing in the project as they transition towards independent living.

Key Responsibilities:

Develop personalized care plans to guide and assist each resident in their journey to independent living.
Provide holistic support to young people, addressing their practical self-care needs, health, budgeting, and domestic skills.
Encourage and build the confidence and self-esteem of young individuals leaving care.
Advocate for residents regarding housing, social work, social welfare, education, training, and legal rights.
Collaborate with and make referrals to relevant external agencies.
Deliver individual support and key-working to project residents.
Facilitate group work sessions for the residents.
Work on a 7-day rotating schedule.
Assist in maintaining high hygiene standards within the project.
Identify and report any Health, Safety, and Welfare risks in the workplace to your line manager or Health & Safety Representative.
Maintain accurate case records and statistics.
Regularly communicate with the Project Leader regarding individual case issues.
Engage in regular supervision sessions with the Project Leader/Assistant Project Leader.

Collaborating closely with a multidisciplinary team and partner agencies, you'll oversee and implement interventions that tackle the fundamental issues preventing these youths from safe living conditions at home or in foster care.This position is not just about leadership—it’s about nurturing a dedicated team, cultivating a safe and supportive atmosphere, and empowering young people to move towards stability and independence.Key Responsibilities:Participate in the development and execution of Care and Placement Plans, ensuring accurate documentation.Champion the rights and responsibilities of each young person within the service.Encourage participation in educational and training opportunities, as well as medical and clinical appointments.Manage personal documents, arrangements, and finances on behalf of the young individuals.Act as a liaison among the young person, their families, and various services.Attend meetings regarding the care and development of the young person.Engage in age-appropriate activities and play with each young person.Implement physical interventions as necessary in accordance with local policies.Account for any expenditures made on behalf of the service, demonstrating effective planning and resource management.Regularly report on the progress of each young individual to the Manager.Document and report any incidents, regardless of severity, involving the young person.Assist in supervising students and other staff as required.

Demonstrated experience in a social care environment. Full clean driver’s license and access to a car (Desirable). Familiarity with conducting assessments, including risk assessments (Desirable). Capability to manage challenging behaviors (Desirable).
