At Clover Health, we are on a mission to revolutionize healthcare and enhance patient outcomes through our groundbreaking primary care tool, Counterpart Assistant. By empowering Primary Care Physicians (PCPs), we facilitate superior patient care at a reduced cost, focusing on early diagnosis and comprehensive management of chronic conditions.

We are currently in search of a Provider Success Manager who will cultivate partnerships with healthcare providers and champion the adoption, growth, and overall success of Counterpart Assistant (CA). This role will oversee a portfolio of provider collaborations, ensuring practices effectively implement and optimize their use of our platform to excel in value-based care.

This position is results-oriented and user-facing, acting as an external product Subject Matter Expert (SME) who promotes overall customer satisfaction. You will employ your expertise in Medicare and Primary Care operations, strong relationship management abilities, and a data-focused strategy to strengthen our market presence and enhance the value we provide to our partners. Candidates must be based in San Antonio and be prepared to travel to practices up to five days a week.

As a Provider Success Manager, your responsibilities will include:

  • Manage Provider Relationships: Serve as the primary liaison for primary care provider partners, establishing trusted, consultative relationships to encourage the adoption of CA.
  • Support Provider Recruitment Activities: Collaborate with the CA Go-To-Market team to implement best practices for provider onboarding and training, focusing on developing efficient in-office workflows and engaging closely with both clinical and non-clinical staff to ensure effective platform adoption.
  • Enhance Provider Success & Retention: Develop and manage a structured engagement plan to ensure providers fully utilize our platform, achieve measurable success in value-based care, and maintain long-term partnerships.
  • Lead Data-Driven Initiatives: Assess provider performance metrics and proactively identify trends, opportunities, and risks with partner practices. Implement strategies to improve provider outcomes and boost platform utilization.
  • Promote Best Practices: Advocate for the adoption of Counterpart Assistant best practices across various PCP settings.
  • Collaborative Team Player: Work alongside internal teams, including sales, product development, and operations, to enhance Clover Health’s offerings based on provider feedback and market insights.
