Key Responsibilities:Oversee patient census using a solutions-focused approach to bridge gaps in both clinical and non-clinical care. Coordinate community resources beneficial to patient care. Educate patients on managing chronic conditions and improving health literacy. Manage medication, including identifying concerns, reconciliation, adherence, and refills. Ensure timely delivery of essential services, including Home Health, DME, and Home Infusion. Address patient inquiries and foster open communication to assess needs. Assist with referrals and scheduling appointments. Requirements:Must hold an active Illinois LPN license. Proficient in Microsoft Office Suite. Strong written and verbal communication skills. Preferred Qualifications:Experience with Epic. Bilingual abilities are a plus. Remote Work Requirements:Reliable high-speed Wi-Fi connection. A designated home office or HIPAA-compliant workspace. Physical Requirements:This is a sedentary position requiring prolonged periods of sitting. Ability to communicate clearly and listen attentively, primarily over the phone. Capable of processing information and taking comprehensive notes during phone conversations.

What You Will Do

  • Coordinate care for patients managing chronic illnesses
  • Identify appropriate resources to support patient needs
  • Collaborate with providers and other care team members
  • Work entirely remotely with no in-person patient contact

Schedule

  • Full-time, Monday through Friday
  • No weekend shifts
  • Rotational on-call duty about twice per year

What We Look For

  • Current Illinois LPN license (required)
  • At least 3 years of direct patient interaction experience

  • Ability to thrive in a changing, high-activity setting
