Qualifications
Key Responsibilities:Oversee patient census with a focus on resolving clinical and non-clinical care gaps. Identify and coordinate community resources that support patient care. Deliver patient education and enhance health literacy regarding chronic condition management. Manage medication protocols, including spotting potential issues, ensuring reconciliation, fostering adherence, and coordinating refills. Facilitate timely service delivery for your patients, covering Home Health, DME, Home Infusion, and other essential needs. Address patient inquiries and foster open communication to fully understand their needs. Assist in managing referrals and scheduling appointments. Essential Qualifications:Active Compact/Multi-State license (LPN) (LVN)Proficient in Microsoft Office SuiteStrong written and verbal communication skillsPreferred Qualifications:Experience with Epic systemsBilingual abilitiesAdditional single state licensesRemote Work Requirements:Must have active high-speed Wi-Fi. Must possess a dedicated home office or HIPAA-compliant workspace. Physical Requirements:This role is primarily sedentary, requiring prolonged periods of sitting. Effective verbal communication and attentive listening are essential, often via telephone for extended durations. The ability to comprehend, process, and take comprehensive notes in real-time during phone calls is necessary.
