Key Responsibilities:Oversee patient census with a focus on resolving gaps in both clinical and non-clinical care. Identify and connect patients to community resources that enhance their care. Educate patients on managing chronic conditions and improving health literacy. Manage medication, ensuring adherence, reconciliation, and timely refills. Facilitate the delivery of essential services including Home Health, DME, Home Infusion, etc. Address patient inquiries and maintain open communication to fully understand their needs. Assist with referrals and scheduling appointments. Additional Requirements:Must hold an active Massachusetts LPN license. Technical proficiency in Microsoft Office Suite. Strong written and verbal communication skills. Preferred Qualifications:Experience with Epic systems. Bilingual candidates are encouraged to apply. Possession of additional LPN licenses in other states is a plus. Remote Work Requirements:High-speed internet connection is essential. A dedicated home office or HIPAA-compliant workspace is required. Physical Requirements:This role is primarily sedentary, requiring prolonged sitting. Ability to communicate clearly and attentively listen during lengthy telephone interactions. Capable of understanding and documenting information in real time during calls.
