Key Responsibilities for a Case Manager:
- Oversee a team of field nurses.
- Conduct supervisory visits in clients' homes as needed.
- Inspire and motivate others to deliver outstanding care.
- Conduct supervisory/assessment visits for our adult and geriatric clients in the Indianapolis, IN area.
- Facilitate new hire orientations and training for incoming nurses.
- Coordinate and/or deliver in-services for field staff.
Qualifications:
- Registered Nurse (RN) with a minimum of 2 years of experience as an RN is required.
- 1+ years of Home Health experience required.
- 2 years of Oasis experience preferred .
- Experience in acute, med/surg, and/or sub-acute settings .
- Supervisory experience is a plus.
