Bilingual RN Case Manager
Location: Onsite in Dubuque, IA. Also accepting remote applicants.
Join our dedicated team at HealthCheck360 as a Bilingual RN Case Manager. We are looking for a caring and meticulous professional to provide exceptional case management services throughout the entire continuum of care. In this role, you will assess, plan, implement, coordinate, monitor, and evaluate care for our consumers, ensuring they receive high-quality outcomes and cost-effective treatment. This position is based in our Dubuque office but is also open to remote candidates.
Key Responsibilities:
- Deliver telephonic case management and utilization reviews for assigned consumers.
- Design, implement, and oversee personalized care plans to achieve quality and economical outcomes.
- Collaborate effectively with healthcare providers, payers, and internal teams to ensure seamless care coordination.
- Act as a liaison between consumers and benefit administrators, promoting clear communication and support.
- Monitor and report on case outcomes, highlighting cost savings and improvements in quality.
Qualifications:
- Bilingual: Proficient in Spanish.
- Education: RN licensure in Iowa required; BSN or higher is preferred.
- Experience: At least 2 years of clinical practice required; experience in case management or utilization review is strongly preferred.
- Skills: Excellent communication, problem-solving, and computer skills with the ability to work independently.
