Family Centre Case Manager - Dublin

As a Family Centre Case Manager, you will play a vital role in supporting families facing homelessness. By leveraging effective case management and assessment techniques, you will empower these families to acquire essential skills that will enable them to maintain temporary accommodation while transitioning to long-term housing solutions.

Key Responsibilities:
Conduct assessments and create/implement tailored support plans for families experiencing homelessness.
Facilitate implementation of support plans to assist families in sustaining emergency accommodations.
Act as an advocate for families, liaising with relevant statutory bodies.
Provide advice, resources, and advocacy to families in need.
Identify the needs of families and contribute to the development of the Family Centre services.
Work under the supervision of project management to ensure effective service delivery.
Collaborate with the team in delivering triage and Family Centre services.
Respond to inquiries via phone, email, and in-person requests for support.
Establish connections with local services and community groups as necessary.
