Enhance Provider Success & Retention: Develop and manage a structured engagement plan to ensure providers fully utilize our platform, achieve measurable success in value-based care, and maintain long-term partnerships. Lead Data-Driven Initiatives: Assess provider performance metrics and proactively identify trends, opportunities, and risks with partner practices. Implement strategies to improve provider outcomes and boost platform utilization. Promote Best Practices: Advocate for the adoption of Counterpart Assistant best practices across various PCP settings.
