Compensation Details:
- $32 - $35 per hour (up to $35 for RNs with over 5 years of experience and a BSN or MSN)
- Shift Differentials for Health Coaches:
- Additional $4/hour for hours worked after 5 PM
- Additional $2/hour for hours worked on weekends
Work Schedule:
- Monday – Friday, 11:30 AM – 8 PM (Shifts may adjust when working from home; potential shifts could be 12:30 PM – 9 PM or 1:30 PM – 10 PM)
- Rotating weekends, working every 3rd Saturday and Sunday with weekdays off.
The RN Health Coach will engage with members regarding their healthcare objectives. Responsibilities include conducting telephonic assessments, creating personalized care plans, and guiding members through their healthcare options. This role encompasses both inbound and outbound calls to foster a coaching relationship with members, providing ongoing disease management coaching and symptom support education.
Daily Responsibilities:
- Deliver unbiased, evidence-based health information to assist individuals with health-related concerns via phone.
- Focus on educating and guiding callers through their healthcare decisions to empower them as informed participants in their healthcare journey.
- Provide general symptom management, urgent question support, chronic condition management assistance, and general health information.
- Utilize approved health coaching tools and resources to educate and guide clients.
- Document calls and general information in the company database for member tracking.
- Conduct assessments as needed and aid callers in creating action plans for specific health conditions.
- Contribute to the development of departmental annual goals and outcomes for continuous quality improvement.
- Seek opportunities for professional development and share knowledge with the Health Coach team.
- Act as a resource and mentor for new employees while identifying ongoing educational needs of the department.
- Participate in case conferences as required with peers and leadership.
