About the Role
The Care Guide serves as a personal resource for Porter members, assisting them with a wide range of healthcare-related needs and facilitating their journey through the healthcare system. Ideal candidates will possess the experience, motivation, and empathy necessary to proactively identify and address member needs, help them understand the intricacies of the healthcare system, maximize their insurance benefits, and find cost-effective solutions to reduce healthcare expenses. Responsibilities include scheduling practitioner visits for in-home assessments, assisting members with their accounts and products, providing personalized service, addressing a variety of healthcare questions, documenting Social Determinants of Health (SDOH), and connecting members with community resources.
