Purpose:
We are seeking a skilled Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative care to homebound patients in the D. C. region on a Full-Time basis. In this role, you will employ medical techniques under physician supervision and cater to a diverse patient population with chronic illnesses.
What You'll Do:
- Deliver exceptional care to primary and palliative patients in and around Prince George County.
- Conduct comprehensive health assessments through physical examinations, medical history reviews, and develop tailored care plans.
- Communicate effectively with office staff and clinical operations across various internal platforms.
- Provide high-quality, patient-centered care to a designated panel of patients.
- Formulate impressions, diagnoses, and treatment plans.
- Build trust and engage with patients, families, and caregivers during visits.
- Order necessary diagnostic tests.
- Prescribe medications and implement appropriate medical interventions.
