Position Overview:

We are excited to expand our team and are looking for a compassionate and skilled Certified Nurse Practitioner to join our Advantmed provider network. In this role, you will conduct in-home wellness assessments for Medicare members and other at-risk populations, focusing on their overall health to ensure accurate risk adjustment coding that promotes better value-based care. This position is 1099 (PRN) providing significant flexibility to accommodate various priorities.

 

Core Responsibilities:

In-Home Clinical Assessments:

  • Travel to patients' residences to perform comprehensive health evaluations.
  • Conduct Annual Wellness Visits and assessments for members with chronic conditions.
  • Execute medical histories and physical examinations, review systems, vital signs collection, and medication reconciliation.
  • Administer preventive health screenings including Hemoglobin A1C, Spirometry, Kidney Health Evaluation, Diabetic Retinal Exam, and FIT.

 

Patient Education and Engagement:

  • Provide preventive health education and counseling to patients.
