Position Overview
The Director of Value-Based Care will spearhead the management of our Medicare ACO programs and oversee the performance of value-based contracts across our expansive national network of NP-led practices. This pivotal role lies at the crossroads of population health strategy, payer performance, clinical transformation, and data-driven execution.
As a key leadership figure in a rapidly growing startup, you will guide and mentor a small yet effective team, including analysts and practice transformation specialists, while also engaging in direct contributions as the team expands. Collaboration with practices, payers, product, and data teams will be essential to design, implement, and scale national population health programs.

Key Responsibilities
Medicare ACO Leadership
* Oversee the daily management and performance of the organization’s Medicare ACO programs.
* Enhance quality, utilization, and financial performance across attributed populations.
* Collaborate with internal and external stakeholders to ensure compliance with CMS requirements and ACO operational standards.
Value-Based Contract Performance
* Direct performance management for Medicaid, commercial, and Medicare Advantage value-based care contracts.
* Analyze and communicate contract performance trends, risks, and opportunities to practices and the leadership team.
* Formulate and implement intervention strategies to enhance outcomes, cost performance, and shared savings results.
Practice & Population Health Strategy
* Work closely with NP-owned primary care practices to facilitate practice transformation, care model integration, and population health workflows.
* Convert performance data into actionable insights for clinicians and practice teams.
* Assist in the deployment and optimization of population health initiatives such as risk stratification, care gap closure, and high-risk patient management.
