Minimum Qualifications:Bachelor’s Degree in Social Work, Human Services, Psychology, or a related field. Social Workers must possess a valid license in Minnesota. A minimum of two years of supervisory or leadership experience. At least two years of experience in waivered case management. Proficiency in Microsoft Office, Google Suite, and care-related software platforms. Knowledge, Skills & Abilities:Exceptional analytical and problem-solving capabilities with a keen eye for detail. Outstanding written and verbal communication skills with the ability to adapt coaching methods to different learning styles. Proven ability to work independently as well as collaboratively within a team environment, engaging with diverse stakeholders.

Join Accord as a dedicated Manager of Case Management (MCM) and lead a passionate team of 13-15 case managers focused on person-centered care. The MCM will drive leadership initiatives, enhance staff retention, and cultivate strong partnerships with both internal and external stakeholders.

This hybrid position requires a minimum of two days in the office each week, based in the Midway area of Saint Paul, MN. The MCM will also travel within the community to engage with staff members, who will work and receive training on-site for their first 90 days.

Key Responsibilities:

  • Supervise and mentor case managers serving program participants, setting performance standards, and tracking progress towards goals.
  • Coach and develop case managers to create and meet established goals and benchmarks.
  • Formulate and implement strategies for productivity and team enhancement.
  • Establish and manage retention and engagement systems to address team concerns.
  • Act as a subject matter expert for case managers, ensuring compliance and high-quality service delivery.
  • Oversee hiring, performance reviews, and termination of direct reports as per established protocols.
  • Organize and lead team meetings with a focus on engagement and collaboration.
  • Ensure timely billing and documentation of services while adhering to best practices and accuracy.
  • Manage budget and expenses, ensuring alignment with staffing and financial expectations.
